Astrana Health (NASDAQ: ASTH), a provider-centric healthcare company, today announced a strategic partnership with Elation Health, a clinical-first technology company specializing in primary care. 

– The partnership focuses on supporting independent physicians by providing them with the tools necessary to thrive in value-based care arrangements. Astrana and Elation plan to establish risk-bearing entities, such as accountable care organizations (ACOs) and clinically integrated networks (CINs), anchored by existing and future Astrana affiliates.

Unified Technology and Care Enablement Solutions

The partnership brings together Elation’s electronic health record (EHR) platform, practice transformation tools, and billing technology with Astrana’s comprehensive value-based care platform and enablement solutions. Astrana’s solutions include:

  • Value-based contracting and credentialing
  • AI-driven population health analytics
  • NCQA-certified HEDIS gaps in care engine
  • Care management and disease management platform
  • Administrative services (billing, coding, call centers)

Putting the Partnership into Action: A Case Study in Hawaii

Astrana and Elation will pilot this new model with a group of over 100 primary care providers serving over 20,000 patients in Hawaii. This provider organization primarily serves Medicare beneficiaries. Astrana will onboard these providers onto its Care Enablement platform by the end of Q3 2024 and act as their exclusive care enablement partner.

This initiative demonstrates Astrana’s commitment to supporting providers in value-based care arrangements. By integrating Elation’s technology with Astrana’s care enablement expertise, the partnership aims to create a model that improves care quality and value for patients in Hawaii.
