RealizedCare will collaborate with health plans, employers, and value-based providers, offering a fully managed solution to identify, assess, and engage members with chronic pain. The platform guides individuals through a personalized care journey, incorporating digital interventions, community support, and direct clinician referrals when necessary.
RealizedCare’s offering includes assigning a pain coach to members, delivering personalized care plans comprising pain neuroscience education, mindfulness practices, calming experiences, and immersive, gamified, graded exercise and movement. Members gain access to PatientsLikeMe’s community of over 160,000 individuals living with chronic pain, providing peer and community support, enriching the care experience.
