StrokeCP: A Collaborative Effort to Optimize Stroke Recovery

StrokeCP leverages insights from the COMPASS study, a comprehensive five-year research initiative conducted across North Carolina.  This data-driven approach ensures that StrokeCP offers effective tools and interventions proven to support post-stroke care management.

Key features of StrokeCP include:

– Personalized Care Plans: The platform utilizes patient-specific data, including clinical history, demographics, medications, and social determinants of health, to create personalized care plans with tailored resources.

– Remote Monitoring and Coaching: StrokeCP facilitates remote patient monitoring, coaching, and seamless communication between patients and care providers.

– Bidirectional EHR Integration: Seamless integration with electronic medical records allows for efficient data capture and utilization.

– Focus on Social Determinants of Health: StrokeCP addresses social factors that can impact recovery, aligning with the increasing focus on social determinants of health (SDOH) in healthcare. CMS now mandates screening for five key SDOH risk factors, which disproportionately impact underserved communities. Research shows that addressing SDOH can significantly improve stroke outcomes, particularly for vulnerable populations.

– Reduced Racial and Ethnic Disparities: By addressing SDOH and offering personalized care, StrokeCP has the potential to reduce existing racial and ethnic disparities in stroke incidence and recovery.
